   Section 4(1)(b)(vi)
 
Statement of the categories of documents that are held by Medical Department or under its control                                                                                               

   The following records are held under its control by Medical Branch for discharge of its  function –

·        Statement of Hurt on duty

·        Statement of leave

·        Statement of Medical Sickness

·        Statement of MCDO s  and PCDO s

·        Statement of Medical Unfit.

·        Records of --  Indoor admission ,Outdoor patients , Operations, Deliveries, Immunisations,  Different investigations, Referral cases, Reimbursements cases, Clinical Meetings,  Specific Disease like Tuberculosis , Cancer , Infectious disease Etc  

·        Other records – Records of tour programmes, Duplicates slips of Medicine , Work programmes /M&P programmes ,General correspondence files, Medical examination records, Medical Board reports, Initial Medical examination reports ,Periodic Medical examination reports, Records of circulated letters on policy matters

·        Accident reports

·        Birth register

·        Death register

·        Death certificate

·        Medico legal case reports.                                                     

                                           Section 4(1)(b)(vii)
The particulars of any arrangement that exists for consultation with, or representation by the members of the public in relation to the formulation of its policy or implementation thereof 

                                       Policy as initiated by Railway Board in regard to the reply to the representation made by the MPs, MLAs and ministers within the specified time limit are maintained in CLW.

   In regard to representation from MP, MLA & ministers the reply must be sent within 30 days after due examination on the part of implementation                                                   

                                             Section 4(1)(b)(viii)
Statement of the boards, councils, committees and other bodies consisting of two or more persons constituted as its part or for the purpose of its advice, and as to whether meetings of those boards, councils, committees and other bodies are open to the public, or the minutes of such meetings are accessible for public
 

There is one elected Staff Council Member in Medical department with whom regular interactions being made by the appropriate authority to sort out the problems relating to medical departments.

Regular interaction with other public organisations, union members of locality are also done time to time.

Monthly P.O.M. meeting are being held by the t H.O.D. s of different departments  with   staff council and its nominated officers and 17 elected members and 1 co-opted SC/ST member  along with other union representatives . In these meetings  agendas related to different departments, including Medical dept. are discussed and solved and guidelines are formulated and implemented at local levels. The minutes of such meetings are circulated in different departments.

 Directory of officers and employees of combined Medical & Public health department. 

Directory of officers and employees of combined Medical & Public health department are maintained bill unit wise by the Pay Bill Section. The Bill units are 840 , 841 , 842

Monthly remuneration received by each of Medical Officers and employees of combined Medical & Public health , including the system of compensation as provided in its regulations

Bill Unit wise monthly remuneration received by each of its officers and employees including the system of compensation viz. HRA, CCA, Washing Allowance, Transport Allowance, Traveling allowance, Contingent Charges as provided in the regulations are being paid and the records are maintained by the Central Pay Bill section of CLW.
